Blakely, bureaucracy is operating on out of date instructions. As bureaucracy tends to do, and politicians tend to be unaware of until a crisis. Institutional malaise is almost inevitable, And often takes a very specific, narrow focused crisis to break free of it

If there is indeed no backup system, that is a very bad thing in life critical systems. Much more needs to be written and exposed.
